- comments: observed a slight debris accumulation encrusted on the interior of the ice machine. Must remove debris, detail clean inside ice machine/have serviced and maintain ice machine at all times.

- comments: observed no consumer advisory disclosure and reminder statement on the menus. Instructed management to disclose which foods can be ordered 'raw' or under cooked and link those foods with an asterisk (*) to a reminder statement that states that eating raw or under cooked foods can increase your risk of food borne illness. Priority foundation violation 7-38-005.
36. Thermometers provided & accurate
- comments: instructed to provide and maintain an accurate working thermometer inside the the refrigerator located in the pizza prep/dishwashing area.
38. Insects, rodents, & animals not present
- comments: observed gap opening at bottom of rear exit door located in the rear pizza prep/dishwashing area. Must pest proof mentioned door all across the bottom to prevent pest entry.

- comments: backflow prevention device not located at ice machine located in the rear pizza prep/dishwashing area. Must install so backflow prevention device may be located to be serviced and maintained.

- comments: observed the following previous core violation not corrected from report #2285957 dated 4/29/2019: #58 ' no food allergen training certificates on premises. Instructed all city of chicago certified food managers must have food allergen training certificates on premises. ' instructed to correct and come into compliance. Priority foundation violation 7-42-090,citation issued.
