- comments: 4-602. 11(b-e)--observed a slight debris accumulation encrusted on the interior of the ice machine. Must remove debris, detail clean inside ice machine/have serviced and maintain ice machine at all times.
38. Insects, rodents, & animals not present
- comments: observed approximately 15 rodent mice droppings on the floor underneath north shelf inside the walk-in-cooler and approx: 20 rodent mice droppings on the floor behind and around the chest freezers at rear storage area directly outside the walk-in-cooler. Instructed to detail clean and remove all rodent mice droppings and sanitize all affected areas,additional pest control service is needed to eliminate the pest activity. Priority foundation violation 7-38-020(a),citation issued.

- comments: observed broken weatherstrip at rear exit door with 1/4' gap opening at bottom of door. Must make door tight fitting all across the bottom to prevent pest entry.

- comments: 4-501. 11--observed a torn door gasket inside of the 3 door prep cooler. Instructed to replace the torn door gaskets and maintain.

- comments: instructed to detail clean and maintain floors with grease and excessive dried food debris accumulation along the walls and in all corners throughout prep/cooking area especially under and around cooking equipments.
