- comments: 2-102. 14(o) observed no employee health policy on site. Instructed to provide. Priority foundation 7-38-010.

5. Procedures for responding to vomiting and diarrheal events
- comments: 2-501. 11 observed no written procedure nor adequate supplies on-site to respond to vomit properly and diarrhea events that can potentially occur inside of the facility. Instructed to provide. Priority foundation 7-38-005.

10. Adequate handwashing sinks properly supplied and accessible
- comments: 5-205. 11 observed the handwashing sink not accessible with bags in the basin of the handwashing sink at the bar and a bowl inside the basin of the handwashing sink in the food prep area. Instructed to never store items inside the basin of the handwashing sink and always keep the handwashing sink accessible for proper handwashing. Priority foundation. Citation issued 7-38-030(c).

- comments: 5-203. 11 observed a hand washing sink is needed at the drink station located outside of the food prep area and an additional hand washing sink in the food prep area by the dish washing station. - comments: 4-501. 112 (a:1 & 2) observed inadequate hot water sanitizing temperature at the high temperature dish machine, at the time of the inspection hot water rinse cycle only reached 140f. Inspector tested dish machine with temperature disc and temperature disc recorded 140f. Instructed to always maintain hot water dish machine at a proper sanitizing temperature of 180f at all times. Priority violation. Citation issued 7-38-025.

- comments: 3-501. 16(a:2,&b) observed time/temperature control for safety foods (2 lbs of cooked chicken) at an improper cold holding temperature of 52f in the food prep area. Instructed to discard and denature said tcs foods and always maintain all tcs cold foods at 41f or below. Priority violation. Citation issued 7-38-005.

- comments: 6-202. 15 observed a wall fan that leads directly outside not screened or protected from pests entry. Instructed to provide a screen or keep the wall fan closed.

- comments: 6-501. 111 observed evidence of pest activity on site. Observed about 20 live cockroaches in between the wok station and the stove. Observed about 20 rodent droppings on the lower shelf of a prep table and about 20 rodent droppings on the ground by the drink prep station. Instructed to clean and sanitize affected areas, eliminate pest activity, and contact pest control for a service. Priority foundation. Citation issued 7-38-020(a).

- comments: 4-101. 17 observed wooden chopping blocks in the food prep area. Instructed not to use wooden chopping blocks for foods, only use smooth and easily cleanable surfaces for all food contact surfaces.

- comments: 4-603. 15 observed pots too large to be submerged in the 3 compartment sink. Instructed to remove large pots or install a larger 3 compartment sink or provide a manual procedure for washing, rinsing, and sanitizing large pots.

- comments: 4-601. 11(c) observed excessive accumulated grease debris in between the stove and wok station and around the oven. Observed the interior of the fryers with excessive accumulated food and grease debris. Observed minor debris on the shelves inside of the walk-in cooler. Instructed to clean all.

- comments: 5-205. 15(a) observed hot and cold water knobs stripped at the 3 compartment sink behind the bar and hot water unable to turn on at the right faucet of the 4 compartment sink and cold water unable to turn on at the left facet of the 4 compartment sink located in the food prep area. Instructed always to maintain plumbing at all times. Priority violation. Citation issued 7-38-030(c).
