- comments: observed no chicago food sanitation manager on site or valid certificate posted when tcs food (chicken 141. 5f, sausage 181. 6f) prepared and served. A chicago food sanitation manager must be on site at all times tcs food prepared and served. Priority foundation 7-38-012 citation issued.

- comments: observed drainpipe missing on hand washing sink in meat prep area. Instructed manager to replace missing drainpipe, repair and maintain hand washing sink. Priority foundation 7-38-030(c) citation issued.

- comments: observed no hand washing signs at hand washing sinks in washrooms, produce, deli, and meat prep areas. Instructed manager to provide hand washing signs at all hand washing sinks used by employees.

- comments: observed rewrapped sliced deli meat, packaged yogurt and sliced melons not properly dated in deli and display coolers. Cold ready to eat tcs food held more than 24 hours shall be dated with discard date within 7 days of preparation. Priority foundation 7-38-005 citation issued

- comments: observed inoperable thermometers in produce display case. Instructed manager to repair and maintain.

- comments: observed over 80 mice droppings scattered on floor along walls and corners near blue totes and office in beverage storage area. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a) citation issued.

- comments: observed food items stored on floor in prep, storage, walk-in cooler and walk-in freezer areas. Instructed manager to elevate all food items six inches off floor in all areas.

- comments: observed soiled wiping cloths stored on prep tables. Instructed manager to store wiping cloths in sanitizing solution between uses.

- comments: observed cutting boards with deep grooves in meat prep area. Instructed manager to replace and maintain.

- comments: observed residue on interior surfaces of ice machine, grease buildup on fryer cabinets and oven, food debris on interior surfaces of coolers, freezers, prep tables, shelving units in prep, sales and storage areas. Instructed manager to clean and maintain.

- comments: observed leaking faucet on 3-compartment sink in produce prep area. Instructed manager to repair and maintain.

- comments: observed no covered waste receptacles in washrooms. Instructed manager to provide.

- comments: observed numerous openings in ceilings in produce prep and beverage storage area, damaged floor tiles, ceiling tiles and peeling paint on ceiling in deli and meat prep area, missing baseboards along walls in meat prep area. Instructed manager to repair and maintain.

Instructed manager to clean and maintain.
