3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: no written employee health policy on site. Must provide for all employees. Priority foundation violation.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed both handwashing sinks in the food prep and dishwashing areas block, not accessible. A large garbage can be stored in front of the prep hand sink and dish hand sink blocked by hanging clean utensils, and a sink filled with stored items such as a first aid kit etc. The manager removed all items blocking sinks. Priority foundation violation 7-38-030(c) citation issued.

16. Food-contact surfaces: cleaned & sanitized
- comments: observed an employee washing dishes at the three-compartment sink that was not properly set to wash, rinse and sanitize. No sanitizer was noted in the third compartment filled with water where washed dishes were then put. Reviewed proper sanitizing at the three-compartment sink. All dishes were re-washed and sanitized. Priority violation 7-38-025 citation issued.

22. Proper cold holding temperatures
- comments: observed tcs foods such as tofu and beans at 49. 5f and potato salad at 45. 5f in two separate coolers. All foods discarded. Priority violation 7-38-005 citation issued.

33. Proper cooling methods used; adequate equipment for temperature control
- comments: observed two coolers unable to maintain adequate temperatures. Front display cooler at 51f and rear cooler at 46. 8f. Both tagged held for inspection and must not use until repaired, maintains a temperature of 41f or below, c. D. P. H contacted for re-inspection and tag removal. Priority violation 7-38-005 citation issued.

37. Food properly labeled; original container
- comments: all pre-packaged prepared foods observed, such as potato salad for sale in the dining room display cooler, must be properly labeled with the business name, address, and list of ingredients.

38. Insects, rodents, & animals not present
- comments: no current pest control service report on the premises for review. Last service report dated 12-14-18. Must provide a current pest control service report. Priority foundation violation.

39. Contamination prevented during food preparation, storage & display
- comments: must not store clean cutting boards at the three compartment sink faucet/nozzle where dirty dishes were being washed. Must store in a clean utensil storage area.

44. Utensils, equipment & linens: properly stored, dried, & handled
- comments: all pots/pans/mixing bowls stored clean on shelving must be inverted. ---must not store dish racks inside the utility service sink.

45. Single-use/single-service articles: properly stored & used
- comments: all stored items throughout the facility, canned goods, utensils, items in rear stairwell must be stored elevated from the floors.

- comments: front prep hand washing sink noted dirty with coffee stain. --- interior front prep oven with food debris. ----extremely dirty interior of the utility service sink. Instructed to clean and maintain all.

- comments: instructed to install a one compartment prep sink in the front food prep area where coffee station, ovens and blenders are used to prepare foods.

- comments: observed dirty mop head stored inside the utility service sink. Must properly store soiled mop heads inverted to properly dry.

- comments: no proof of training or certificates for food handler training for employees on site. Must provide.

- comments: no proof of allergen training or certificate on site for the above city of chicago certified food manager. Must provide.

60. Previous core violation corrected
- comments: previous core violation from report #2305203 from 8-23-19 not corrected. #55- 6-201. 12- excess debris(dust,food etc) on floor throughout the premises under shelves and equipment. Clean and maintain. Priority foundation violation.
