1. Person in charge present, demonstrates knowledge, and performs duties
- comments: pic doesn't have a certified food managers certificate. Pic arrived on site (priority foundation 7-38-012)(cos)

2. City of chicago food service sanitation certificate
- comments: observed no certified food manager on duty while tcs foods are being prepared,handled and served such as (steak,rice,chicken,diced tomato,etc)must be on site at all times. Manager arrived on site at 3:02 p. M. (priority foundation 7-38-012)(citation issued)(cos)

8. Hands clean & properly washed
- comments: observed poor hygienic practices by a female food handler talking on a cell phone and with plastic gloves on and then removed a burrito from the grill without washing her hands. Instructed to wash hands after every possible contamination. (priority 7-38-010)(cos)(citation issued)

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no soap at hand sink in basement rear prep area and staff toilet room. Soap was provided at hand sinks during inspection. Must provide and maintain. (priority foundation 7-38-030(c)(cos)(consolidated violation)

- comments: observed multi-use utensils being sanitized at 0. 0ppm's at 3 3-compartment sink. Observed female food handler wash dishes with a soapy towel and rinse under running water and place dishes on rack without properly sanitizing dishes. Instructed to set up sink properly to sanitize multi-use utensils. (priority 7-38-025)(cos)(citation issued)

- comments: observed evidence of 4- live roaches crawling on floor in basement rear prep area, in crack and crevice along wall base in rear dish washing area near hand sink. Also, appx. 25 or more live small flies flying around in basement dish washing and prep area. Recommended to have a pest control operator to service premises on or after 9/17/25. - comments: must provide a splash guard in between hand sink and 2- compartment sink in basemnet prep area.

- comments: observed improper usage of gloves by a food handler.

- comments: must clean grease and food debris build up from all used and unused equipment and cooler,walk in cooler shelving units,interior panel of ice machine.

- comments: observed toilet flushing handles missing on toilets in staff and customers' toilet rooms must provide and maintain. (priority foundation 7-38-030(c)(citation issued)

- comments: must repair or replace damaged floor tile in 1st fl. Prep area and where needed,missing wall bases throughout basement prep and dish washing areas,stained and missing ceiling tiles in basement.

- comments: previous core violations from report #2609332 12/17/24 not corrected- #38-6-202. 15 observed a 1/2" gap along the bottom of the front doors. Management instructed to rodent proof all outer openings. #51-5-205. 15 repair the leaks under the 3-component sink in the dish-washing area. Seal the openings in the top grate for the in-ground grease trap. #57-2-102. 13 observed several food handling employees without a food handler certificate. Management instructed that all food handling employees must show proof of training. (priority foundation 7-42-090)(citation issued)
