- comments: premises has no clean up procedure or supplies for responding to vomiting and/or diarrheal events. Instructed manager to provide. Priority foundation violation #7-38-005

- comments: observed no hand washing signs at hand washing sinks in prep area. Instructed manager to provide.

- comments: observed over 50 mice droppings scattered under cardboard on front counter, along ledges of rear prep table, on bottom of prep tables in front prep area, on floor in unused walk-in cooler. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a)

- comments: observed food items stored on floor in prep and storage areas. Instructed manager to elevate all food items off floor.

- comments: observed accumlated grease buildup and food debris on interior and exterior surfaces of cooking equipment, shelving units, prep tables, holding units, coolers, and freezers. Instructed manager to clean and maintain.

- comments: observed stained and missing ceiling tiles in prep, washroom and storage areas. Instructed manager to repair ceiling and replace stained ceiling tiles.
