- comments: observed no employee health policy in place. Instructed manger to comply with the new code or citations will follow. Priority foundation violation #7-38-010.

- comments: premises has no procedures or supplies for responding to vomiting and/or diarrhea events. Instructed manager to comply. Priority foundation violation #7-38-005.

- comments: observed no paper towels of other hand drying provisions at handsink in meat processing area and in employee breakroom area. Instructed manager that all handsinks must be maintained at all times. Priority foundation 7-38-030(c) citation issued.

- comments: observed no thermometer displayed at fresh produce reach in cooler. Instructed manager to provide and maintain.

- comments: observed cutting boards in meat prep area covered with deep grooves and dark stains. Instructed to repair or replace and maintain smooth, cleanable surfaces.

- comments: observed accumulated grease and food debris on interior and exterior surfaces of unused cooking equipment and shelving units in designated employee break area. Also meat display cooler is dirty and stained. Instructed manager to clean and maintain. (nonfood contact surfaces)

- comments: observed no covered waste receptacle in womens toilet room in basement. Instructed manager to provide and maintain.

- comments: observed cracked and dirty floors in rear near walk in coolers and underneath grocery display units throughout. Observed holes/damaged wall surfaces in meat prep area. Observed peeling paint in milk storage walk in cooler. Instructed manager to repair and mainain.
