- comments: observed improper cold temperatures of ( tcs) foods inside prep cooler. Approx 5-lbs of cooked ham off the bone at 51. 0f,3-lbs of cole slaw at 50. 0f,2-lbs raw eggs in container at 59. 0f, 5-lbs of bolied eggs at 54. 0f,2-lbs of sliced tomatoes at 52. 0, 6- t-bone steaks at 57. 0f. Instructed to maintain cold (tcs) foods at 41. 0f or below. All foods was removed and discarded by pic at cost approx $60. 00. Priority violation issued 7-38-005

25. Consumer advisory provided for raw/undercooked food
- comments: observed still no disclosure/reminder for consumer advisory on menu. Instructed manager to provide and maintain. Priority foundation violation issued 7-38-005

33. Proper cooling methods used; adequate equipment for temperature control
- comments: observed prep cooler in kitchen maintaining improper cold temperature of 56. 0f. Cooler must maintain cold temperature of 41. 0f or below. Cooler will be taged held for inspection. . Cannot use cooler until tag is removed by the health dept. Instructed to used additional prep cooler or other coolers on site that 41. 0f or below

37. Food properly labeled; original container
- comments: observed bulk containers not labeled in rear prep area. Instructed manager to label all bulk containers with common name of food.

51. Plumbing installed; proper backflow devices
- comments: observed left faucet handle in womens washroom in poor repair,( hard to stay open). Must repair.

55. Physical facilities installed, maintained & clean
- comments: observed accumulated food debris on floors under cooking equipment and prep coolers, excess water on floor in corners in dish washing area. Instructed manager to clean floors and maintain
