- comments: observed raw eggs being improperly stored in several areas in the walk-in cooler. Raw eggs stored directly above ready-to-eat salad dressings. Raw eggs are stored inside a pan of ready-to-eat prepared lasagna. Foods discarded. Reviewed proper food protection and storage of raw and ready-to-eat foods. Priority violation 7-38-005 citation issued.

19. Proper reheating procedures for hot holding
- comments: observed previously cooked tcs baked potatoes wrapped in tinfoil being reheated in a metal pan inside the steam table. Potatoes were noted at a temperature of 69. 2f. All discarded. Reviewed proper reheating methods of previously cooked tcs foods. Priority violation 7-38-005 citation issued.

21. Proper hot holding temperatures
- comments: observed baked potatoes in the above mentioned hot holding unit at an improper temperature of 62. 9f. Tcs foods must be held hot at a minimum temperature of 135f. Priority violation. See the above violation #19 for citation.

- comments: observed tcs cooked ham in a large, covered plastic container inside a tall reach-in cooler at improper temperatures of 46. 6f and 46. 2f. All discarded. Priority violation 7-38-005. See above violation #19 for citation.

- comments: noted prep area cutting board in extremely poor repair with large open breaks and cracks. Management discarded.

- comments: no chlorine chemical test strips on-site to monitor chlorine sanitizing concentration at the three-compartment sink. Must provide. Priority foundation violation 7-38-005 citation issued.

- comments: must not use tin foil as liner/back splash for equipment.

- comments: observed outside waste containers with overflowing bags of food waste garbage. Also noted are piled bags of dirty, used empty cans of tomatoes at the rear kitchen door. Open garbage containers, plastic buckets, and a shopping cart filled with food waste. Instructed to remove all garbage/food waste from the grounds and open containers. Remove all overflowing garbage from the waste container. Priority foundation violation 7-38-020(b).

- comments: noted pooling water from an unknown source under the three-compartment sink. Must remove and maintain dry.

- comments: the gas stove on the cooks line must be completely under the ventilation hood system.

- comments: no proof of food handler training or certificates for all employees on site. Must provide.

- comments: no proof of allergen training or certificates on site for all the city of chicago certified food managers. Must provide.
