- comments: 6-501. 111 observed evidence of live insect activity. Observed about 25 small flies and about 4 large flies on the walls, ceilings and flying around in the bakery food prep area. Instructed to eliminate insect activity and clean and sanitize affected areas. Recommended contacting pest control for a service. Priority foundation. Citation issued 7-38-020(a).

- comments: 5-205. 15 observed a loose hand washing sink in the meat prep area and a loose faucet in the men's employee toilet room. Also one of two hand washing sink water is not staying on for at least 15 seconds. Instructed to repair all.
55. Physical facilities installed, maintained & clean
- comments: 6-501. 12 observed accumulated debris under the 3 compartment sinks and standing water around the high temperature dish machine. Instructed to clean.
