- comments: observed a food handling employee handle ready-to-eat food with their bare hands. Observed a food handling employee transfer cooked rice with a bowl from the rice cooker into a stainless steel pan and smooth out the rice with their bare hands. Management instructed to discard the contaminated food. Priority 7-38-010. Citation issued.

- comments: observed dead insects on the floor in the basement. Management instructed to clean all dead insects from the floor in the basement.

- comments: observed evidence of rodent activity and live insect activity in the basement. Observed 15 rat droppings on the floor in the basement and in a metal storage cabinet. Management instructed to clean and remove all rodent droppings from the premises. Also observed 1 live cockroach on the floor in the basement next to the water heater. Additional pest control service is needed to eliminate the pest activity. Priority foundation 7-38-020(a). Citation issued.

- comments: clean the top and bottom shelves of the prep tables, stove, wire storage racks in the walk-in cooler and the chest freezers in the basement.
