On June 11, 2018, I went to dinner with 7 other people. Three of us ordered sushi rolls while the others had noodles and other menu items. We did not drink alcohol, only water. My sushi contained shrimp and crab and tasted funny almost immediately after I took my first bite. The fish tasted old, like it the roll had been wrapped up for at least a week before they served it. We left and I headed home around 10:00 PM

By 1:00 AM on June 12, 2018, I was in the bathroom, violently vomitting and had frequent diarrhea. This was often and continuous all through the day and night of June 12. I could not keep down any kind of food or liquid of any kind, as it would lead to more vomitting.

By the morning of June 13, 2018, I could no longer focus or concentrate on anything. I was physically weak and pale and had no liquids left in my body. I had to be taken to the Joliet Fire Department, where I was given the largest IV bag they had in stock. I recovered but could not eat sushi for months.

I had not reported this immediately because I could not locate the appropriate contacts or department to report my incident. | Symptoms: Nausea, Diarrhea, Vomiting, Weakness
