A dead mouse, dirt, and debris were found under equipment in the food prep area; mouse droppings found on the cutting board under slicer; a cat and cat food were found in the rear storage areas; grease seen dripping from grease trap over grill with the potential to contaminate food on the flat top grill; stagnant water observed on the floors in the warewashing area; heavy grease accumulation and food debris was found on the floors, walls, and ceilings in the food prep area; food including milk, turkey bacon and eggs, held at improper temperatures; the establishment did not have a garbage grinder; the establishment was storing, distributing, holding for service, using in preparation of menu items or serving food items containing artificial trans fat; alteration or construction began prior to plan submission and approval.
Due to imminent health hazards observed during this inspection, the establishment was issued a Cease Operations Order and must discontinue food operations immediately. The establishment could not operate for a minimum of 48 hours.
