- comments: observed no consumer advisory disclosure and reminder on menu for consuming raw and under cooked foods. Must provide and mainatin. (priority foundation 7-38-005)

- comments: must label food storage containers when food is not in original package.
38. Insects, rodents, & animals not present
- comments: observed evidence of appx. 75 or more rat droppings on floor under 1- door glass reach in cooler ,inside and under fryers,under prep tables in front prep area,on landing and stairs leading to basement,dining area near umberella,on basement floor and upper landing. Must remove droppings. Clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 5/27/21. (priority foundation 7-38-020(a)(citation issued)

- comments: observed clean dishes piled up inside of 1- compartment sink. Must store & invert multi-use utensils.

- comments: observed a worn and pitted discolored cutting board. Must repair or replace.

- comments: must provide a drainboard at 3- compartment sink.

- comments: must clean excessive grease and food debris build up fryer cabinets,grill table,prep tables,all coolers ,rolling carts,steam table.

- comments: observed hand washing sink in front prep area in poor repair. Observed hand washing sink cold water faucet spinning around when water is turned on and no water comes out. Water is turned on and off by shut off valve underneath sink. Must use additional hand sinks on site for hand washing. Must repair and maintain. (priority 7-38-030(c)(citation issued)

- comments: must provide a lid for garbage can in women's toilet room.

- comments: observed no outside garbage dumpster on site. City dumpsters are being used for garbage disposal. Must provide a commercial dumpster and maintain. (priority foundation 7-38-020(b)(citation issued)

- comments: must repair or replace damaged floor tiles on stairs leading to rear prep/dish washing area,holes in concrete floor in basement,missing wall tiles in prep areas & men's toilet room,damaged wall above food storage leading into basement. Must re-caulk hand sink in women's toilet room.

- comments: observed a massive amount of clutter of articles (brooms,mops,mop buckets,carts,etc) all over rear yard and near door with rat dropping mixed in between. Possible harborage area for rodent and basement landing. Must remove and maintain.
