22. Proper cold holding temperatures
- Comments: Observed the following cold TCS food items at improper temperatures stored inside 2 door cooler with cold hold attached at improper temperatures during time of inspection. They are as follows: Approximately 5 lb sliced tomato, 6 lbs corndogs, 3 lbs pizza puffs, 10 lbs beef, 5 lbs special sauce, 3 lbs coleslaw, 30 lbs (beef hotdogs/polishes), 10 lbs chicken, 5 lbs hamburger. Noted temperature of all items ranging between 55°F–61°F. Instructed to hold all cold TCS foods at 41°F and below and maintain. Priority violation. PIC voluntarily discarded all said items at this time. Total cost $500. Priority violation. 7-38-005. Citation issued.

33. Proper cooling methods used; adequate equipment for temperature control
- Comments: Observed prep cooler (2 door unit, with cold hold attached) at improper cold holding temperatures during time of inspection while storing TCS foods (chicken, corndogs, hamburger, hotdogs, sliced tomatoes). Noted unit temperature at 60°F at this time. Instructed to repair said unit to required 41°F or below as required. Unit tagged and held for inspection. Priority violation. 7-38-005. Citation issued.

39. Contamination prevented during food preparation, storage & display
- Comments: Observed food items stored on floor in storage/prep space. Instructed to provide adequate shelving units and elevate (onions, potatoes, etc.) at least 6 inches off floor to prevent contamination.

54. Garbage & refuse properly disposed; facilities maintained
- Comments: Observed outside dumpster not in compliance. Noted lid with large hole in middle. Instructed to provide tight fitted, durable lid able to prevent feeding source for pest. Must maintain. Priority foundation violation. 7-38-020(b). Citation issued.

56. Adequate ventilation & lighting; designated areas used
- Comments: Observed vent above ice cream prep station with dust and dirt build up. Instructed to clean and maintain.

57. All food employees have food handler training
- Comments: Observed 1 of 2 food handler certificates for food employees. Instructed to provide and maintain on site for review.

58. Allergen training as required
- Comments: Observed City of Chicago certificate holder during inspection without allergen training certification. Instructed to obtain and maintain.

60. Previous core violation corrected
- Comments: Observed previous core violation not corrected from report 2617789 May 20th, 2025. Violation is as follows: 6-201.11 Observed chipped paint on the walls in the storage room/utility sink area. Noted concrete floor paint coating is also peeling/chipping. Instructed to resurface and repaint chipped areas to ensure walls and floor are sealed, smooth and easily cleanable. 6-201.13 Observed wall base in toilet room peeling off wall. Instructed to repair and maintain. Noted prep table added in front of said wall during this time. Instructed to correct and maintain. Priority foundation violation. 7-42-090. Citation issued.
