- comments: observed supplies missing from vomit and diarrhea clean up kit. Must provide all necessary supplies as outlined in policy. Priority foundation. 7-38-005.

- comments: observed approximately 30 rat droppings found throughout establishment. In dining room: 3 rat droppings found behind pepsi cola cooler, 2 rat droppings found behind ice cream cooler and 5 rat droppings found behind card board boxes. 20 rat droppings were found under 3-compartment sink and around grease trap in prep area. Approximately 8 cockroaches found under milk crates that were stored on grease trap and approximately 20 ants were found crawling in crevice of grease trap. Approximately 6 mice droppings were found along wall next to cooking fryer. Must eliminate all evidence of pest activity and detail clean areas. Instructed to contact a licensed exterminator to provide insect and vermin services to treat issue. Priority foundation violation. 7-38-020(a). Citation issued.
