- comments: must label food storage containers when food is not in original package.

- comments: observed evidence of appx. 30 or more mice dropping in lower cabinet at service counter in between clutter. Must remove droppings,clean and sanitize areas. Recommended to have a pest control operator to service prmises on or after 8/5/21. (priority foundation 7-38-020(a)(citation issued)

- comments: observed an appx. '1/2' gap in center of double entry doors. Must make tight fitting.

- comments: must repair or replace worn door gasket on front prep cooler.

- comments: must clean grease and food debris build up from pipes of cooking equipment,rolling carts,dollies,walk in cooler fan guard covers.

- comments: must repair or replace leaky pipe underneath 3- comp sink and leaky faucet at same,leaky ice bin at drive thru counter.

- comments: must provide a garbage can with lid in women's toilet room.

- comments: observed outside garbage dumpster area ground with grease and trash on ground in corral. Must remove trash and grease. Clean and maintain area. (priority foundation 7-38-020(b)(citation issued)

- comments: must repair or replace stained and missing ceiling tiles in dining area,damaged wall panel inside of walk in cooler. Must grout floor in rear prep area and where needed.

- comments: previous core violations from report #2454037 10/30/20 not corrected#49- must clean and maintain the following: exterior surfaces of all cooking equipment, interior of hot holding equipment, prep tables and steam tables, bottoms of fryers and shelves inside of walk-in units, interiors of refrigeration units. #55- the floors and walls around behind and under kitchen equipment in need of cleaning; must clean and maintain to remove grease, food debris, etc. Also clean all ceiling vents in kitchen where needed. #56- instructed to clean the vent system above the cooking equipment.
