- comments: observed no valid city of chicago food service manager certification on site for food service manager. Instructed to obtain and provide on site. Priority foundation violation 7-38-012 citation issued.

38. Insects, rodents, & animals not present
- comments: observed evidence of pest activity. Apprx 10 droppings along the wall on the floor near the ice machine. Apprx 20-30 droppings along the front cash register area under the counter and inside the counter area. Approx 20-30 droppings under the soda machine used for customers and approx 10-15 droppings under the soda machine located at the drive through window. Instructed to contact pest control and have a service with service log upon return inspection dated apr 26th 2022. Instructed to clean and maintain all areas. Priority foundation violation 7-38-020(a) citation issued.
