- comments: observed person in charge without proof of food safety management training during time of inspection. Must provide.

- comments: observed no city of chicago foodservice manager certificate on site during time of inspeection while preparing and serving time/temperature control for safety food (beans, rice, chicken, salsa , etc) instructed to provide and maintain at all times with valid original certificate posted for public view. Priority foundation violation. 7-38-012.
38. Insects, rodents, & animals not present
- comments: observed live roaches in facility in front and rear food preparation areas. Instructed person in charge to inform pest control service, remove all live roaches and sanitize all areas. Priority foundation 7-38-020(a)

- comments: observed dead roaches inside of equipment in taqueria prep area, in rear prep area under tables and on floor and equipment. Instructed person in charge to remove all dead insects and sanitize all areas.

- comments: observed build up of residue and food debris on tables, cooling equipment, and shelving units in front and rear prep area. Instructed person in charge to clean and maintain.
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no covered receptacle in unisex restroom for sanitary products. Instructed person in charge to provide covered receptacle in all washrooms for females use and maintain.
60. Previous core violation corrected
- comments: observed previous core violations from report #2453692 dated:10/22/2020 not corrected: #56;observed inadequate lighting in prep area. Must increase and maintain. #57;observed no food handler certificates on site for all food handlers. Instructed to meet requirement. #58;observed no allergen training certificates on site. Instructed to meet requirements. Instructed person in charge to come into compliance with all previous violations and maintain. Priortiy foundation 7-42-090.
