- comments: observed a container of raw chicken stored next to open container of sliced cabbage and bunches of cilandro inside 2-door prep cooler. Instructed manager to store food items according to food code to prevent contamination. Priority violation 7-38-005. Citation issued.

- comments: found evidence of roach activity inside premises. Found four live roaches crawling on floor behind 3-door tall refrigerator in front prep area. Instructed to clean and sanitize affected area and wherever needed. Additional pest control service is needed to eliminate pest activity. Priority foundation violation 7-38-020(a). Citation issued.

- comments: observed employee not wearing hair restraint while food is being prepared and served. Must wear hair restraints at all times food is prepared and served.

- comments: observed wiping cloths stored on top of prep tables. Must properly stored wiping cloths in a sanitizing solution when not in use.

- comments: no chemical test strips available on premises to measure the concentration of sanitizer at the 3-compartment sink. Instructed to provide chlorine test kit and maintain priority foundation 7-38-005. Citation issued.
