- comments: observed no disposable paper towels for hand drying at handwashing sink in employee all gender washroom. Corrected on site disposable paper towels for hand drying at handwashing sink in employee all gender washroom provided at handwashing sink before conclusion of inspection. Priority foundation violation 7-38-030(c) citation issued
38. Insects, rodents, & animals not present
- comments: observed opening at base of left front entrance door. Instructed to seal all outer openings to prevent pest entrance.
39. Contamination prevented during food preparation, storage & display
- comments: observed 2 liters of soda stored on pallets in washroom. Instructed food may not be stored in locker rooms, toilet rooms, garbage rooms, mechanical rooms, under sewer lines that are not shielded, leaking water lines, open stairwells or under sources of contamination. Instructed to relocate 2 liters of soda stored in washroom to an appropriate area for storage in facility.
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed cardboard used as shelf liner throughout facility on sales floor on shelves for candy, dips/salsa and chips, in cold holding units and in walk in coolers. Instructed to remove cardboard shelf liners throughout entire facility non food contact surfaces shall be constructed of a corrosion-resistant, nonabsorbent and smooth material. Observed beverages and boxes throughout facility stored on floor, soda crates or pallets. Observed liquor and boxes stored on floor in cashier enclosure, observed soda, liquor and beer stored on soda crates, pallets or floor throughout facility in both rear dry storage area, sales floor and in walk in cooler. Instructed to eliminate usage of soda crates and pallets for elevation, instructed to provide adequate shelving units to elevate all food and non food items at least six inches off of floor for ease of cleanability.

- comments: observed damaged floor tiles in washroom. Instructed to smoothly repair all damaged floor tiles in washroom for ease of cleanability. Observed stained and damaged ceiling in large rear dry storage area near ceiling vent. Instructed to to remove stain and smoothly repair ceiling for ease of cleanability in large rear dry storage area near ceiling vent and maintain.

- comments: observed debris and dirt on floors underneath shelving units and pallets on sales floor, walk in cooler and both rear dry storage areas. Instructed to remove debris and clean floors on sales floor, in walk in cooler and in both rear dry storage areas and maintain. Observed black buildup on interior of utility sink. Instructed to remove buildup, clean interior of utility sink and maintain.

- comments: observed wet mop drying in interior of utility sink. Instructed to install mop drying rack for proper drainage and drying of mops to prevent pest breeding.
