- comments: observed 4 live roaches inside cookline 2-door reach in cooler door rubber gaskets and 1 live roach on wall behind coffee machine. Instructed manager must remove all insect activity by date of reinspection. Advised to call pest control operator for service. Priority foundation 7-38-020(a).

- comments: observed food handlers not wearing hair restraints. Instructed manager all food handlers must wear hair restraints.

- comments: observed ice scoop improperly stored on top of ice machine. Instructed manager must provide a clean container for ice scoop.

- comments: prep area seafood 2-door reach in cooler door rubber gaskets ripped. Must repair/replace damaged door gaskets.
