- comments: pic doesn't have a certified food managers certificate. Must provide and maintain. (priority foundation 7-38-012)(no citation issued)

2. City of chicago food service sanitation certificate
- comments: observed no certified food manager on duty while tcs foods are being prepared and handled such as (pizza dough,pizza sauce,etc) must be on site at all times. Manager arrived on site after inspection was conducted and citations written at 12:26 p. M. (priority foundation 7-38-012)(citation issued)

3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: observed no signed employees health policies. Must provide and maintain. (priority foundation 7-38-010)(no citation issued)

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no hand washing signs at hand sinks in prep /dish washing areas and toilet rooms. Must provide and maintain.

36. Thermometers provided & accurate
- comments: must provide a thermometer for 1- door glass reach in cooler.

38. Insects, rodents, & animals not present
- comments: observed evidence of appx. 100 or more mice and rat droppings on floor next to and behind small reach in freezer at call order area,on floor in between 1- door reach in cooler and unused cooler,on pipe behind same cooler,hot water tank area,rear garage area in between articles on floor and along wall bases,under pizza prep counter. Must remove droppings. Clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 1/15/21. (priority foundation 7-38-020(a)(citation issued)

- comments: observed an appx. '1/2-3/4' gap along bottom of garage overhead door. Must make tight fitting.

- comments: must invert multi-use utensils on dish storage shelves.

- comments: must clean flour,food debris and dust build up from prep tables, interior and exterior of all cooking equipment,fryer cabinets,steam tables,prep table,potato peeler,unused equipment,wheel bases,can cart,icee machine,hot holding units,raech in cooler. Must defrost small reach in freezer.

- comments: must repair or replace low cold water pressure at hand sink in pizza prep area,leaky faucet and pipe at 3- compartment sink.

- comments: must replace broken self closing device on front toilet room door.

- comments: must clean excessive black build up on wall over 3- compartment sink near potato peeler,all prep areas.
