10. Adequate handwashing sinks properly supplied and accessible
- comments: handwashing sink not installed in prep area/kitchen where cooked and raw foods are handled. Instructed to install said sink at said area. Priority foundation violation:7-38-030(c), no citation issued.

16. Food-contact surfaces: cleaned & sanitized
- comments: found low temperature chemical dish machine not dispensing chlorine chemical sanitizer, after several attempts still 0ppm. A dish rack inside the unit with washed utensils: plastic bottles, strainer, cups, bowls etc. Dish machine tagged held for inspection and must not use until repaired and properly dispenses chlorine sanitizer of 100ppm. Tagged unit 'held for inspection' do not use. Use three compartment sink. Two days given. Priority violation 7-38-025. Citation issued. On 8-12-20 chlorine chemical sanitizer solution provided at low temp dish machine. Presently 100ppm.

25. Consumer advisory provided for raw/undercooked food
- comments: observed no consumer advisory disclosure or reminder on raw under cooked foods on menu(sushi) must provide and maintain, on computer shows consumer advisory however no disclosure (by the food items) for all under cooked foods/sushi. On carry out and delivery menu no consumer advisory, disclosure or reminders to the customers. Instructed to have a consumer advisory on the menu with a disclosure notice and reminder statement. Priority foundation violation 7-38-005 citation issued. On 8-12-2020 manager provide disclosure on menu on computer,and all other required documentation.

29. Compliance with variance/specialized process/haccp
- comments: found sushi rice lab letter test has expired. Instructed manager must update sushi rice lab letter on premises, must be within 5 years. Priority foundation 7-38-005. No citation issued.

- comments: dry food inside plastic white container must be labeled with common name.

- comments: waste pipe about food and food products in basement area must be encased or relocate food items
