- comments: observed evidence of pest and insect activity. In rear storage area near the toilet room observed approx 20-25 droppings. In the exit room next to the atm machine in the rear observed approx 15-20 droppings. In the front on the sales floor observed approx 5-10 droppings behind the chip racks. In the rear deli meat cooler area observed live activity of 2 roaches, in the basement area observed approx 100-150 rodent droppings. Instructed to contact pest control and have a full service with a service log sheet up return date of 11/09/2022 must clean and maintain all areas. Priority foundation violation 7-38-020(a) citation issued.

- comments: observed the old three compartment sink that is no longer in use with old grease and dirt build up inside casuing very nasty oder. Instructed they must clean and maintain until item is removed from facility.

- comments: observed leak at the rear hand washing sink in the deli area at the drain pipe. Instructed to repair and maintain.
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no covered receptacle in rear toilet room, instructed to provide and maintain.

- comments: observed broken spring on rear toilet room door. Instructed to repair and maintain in good working order.

- comments: observed floors in the rear storage area, the exit room area and the basement area with a build up of dirt, dust and debris. Instructed to clean and maintain all areas.

- comments: observed clutter in the rear storage area, inside the exit room area and in the rear deli area (shelving, refrigeration units, cabinets and other items) instructed to remove all as a means of preventing pest and insect harboring.
