- comments: observed pest activity on premises. Over 10 rat droppings inside the washroom and storage areas, under shelves by the toilet bowl,etc. Remove droppings, clean and maintain. Priority foundation violation: 7-38-020(a), citation issued

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: must not use duct tape as a means of repair at chest freezer door by the butcher's area. Remove, surface must be smooth,cleanable and non-absorbent material.

- comments: must not use cardboards as liner on shelves throughout the selling and storage area, remove. Surface must be smooth,cleanable and non-absorbent material

- comments: replace broken stoppers at three compartment sink in butcher's area handwashing sink slowly drainning at butcher's area. Leaking pipe under the handwashing sink. Instructed to repair cause
