Chicago, 60632 Illinois, United States

Inspectors found several violations during the inspection, including significant food safety violations. Cold foods that require temperature control, including shrimp, chopped lettuce salad, and sliced tomatoes, were being held between 48.0°F and 49.1°F, above the required 41°F or below. The manager voluntarily discarded about 15 pounds of food valued at $115, and a citation was issued.

Inspectors also found vacuum-sealed seafood in the walk-in freezer without the required approved food safety plan for this specialized process. The vacuum-sealing machine, materials, and 75 pounds of vacuum-packaged food were tagged during the inspection, and a citation was issued for the combined violation. Other findings included missing handwashing signs in restrooms, gaps at exterior doors that could allow pests to enter, a missing splash guard at a handwashing sink, improperly stored utensils, rusted or lined shelving, leaking sink drainpipes, a missing backflow prevention device on the ice machine, restroom fixture issues, poor lighting, a stove not fully under ventilation, and incomplete allergen training records for some certified food managers.
