- comments: observed cold tcs foods, held at improper temperatures ranging between 46. 2-52. 0f. Observed cold tcs foods, chicken(50. 5-52. 0f), fish(48. 0-50. 0f) and beef(46. 2-47. 5f)held at improper temperature. Instructed all cold tcs food must be held at 41f or below. Corrected on site manager voluntarily discarded and denatured food valued at $675 weighing 46lbs. Priority violation 7-38-005 citation issued

25. Consumer advisory provided for raw/undercooked food
- comments: observed some animal derived foods that can be served raw, undercooked or cooked to order with no asterisk identifying them as such. Instructed to add an asterisk to all animal derived foods that can be served raw, undercooked or cooked to order. Observed disclosure on menu not stating that some asterisked animal derived foods are served raw and undercooked. Instructed to update disclosure on menu to state that asterisked animal derived foods are served raw, undercooked or cooked to order. Priority foundation violation 7-38-005 no citation issued

33. Proper cooling methods used; adequate equipment for temperature control
- comments: observed two drawer cold holding unit underneath grill at an improper ambient temperature of 53. 4f. Instructed all cold holding units must be maintained at 41f or below. Corrected on site held for inspection tag placed on two drawer cold holding unit underneath grill. Priority violation 7-38-005 citation issued

38. Insects, rodents, & animals not present
- comments: observed openings at base of both outer glass entrance doors on to parking lot on archer/komensky side of facility and rear exit door of prep area. Instructed to seal all outer openings to prevent pest entrance.

39. Contamination prevented during food preparation, storage & display
- comments: observed exposed ice in ice bin in bar with no lid/cover on ice bin. Instructed to provide lid/cover for ice bin in bar to prevent contamination of ice from exposure to splashes, dust or other contaminationin in ice bin during storage.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed equipment and boxes stored on floor, observed pans stored on floor in statging area outside of dressing room and observed boxes stored on floor throughout bar. Instructed to provide adequate shelving units to elevate all food and nonfood items at least six inches off of floor for ease of cleanability.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed doors of northside all gender washroom and northside men's washroom to not be self closing. Instructed to install self closing devices on northside all gender and northside men's washroom doors so that washroom doors shall be tight fitting and self closing.
