- comments: observed shrimp being thawed at room temperature in prep area. Instructed person in charge that all thawing must occur under cold running potable water or refrigeration.

36. Thermometers provided & accurate
- comments: observed no ambient air thermometer in cold hold units in prep area. Instructed person in charge to provide thermometers for all cold hold units and maintain.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed prep coolers in kitchen area on hot line with torn and damaged gaskets. Instructed person in charge to replace and maintain all damaged gaskets.

49. Non-food/food contact surfaces clean
- comments: observed excessive build up of rust on all shelving in walk in cooler where seafood is held. Instructed person in charge to replace/repair and maintain.

51. Plumbing installed; proper backflow devices
- comments: observed leak at faucet of two compartment sink in prep area being held with towel. Instructed person in charge to properly repair and maintain.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no lid on toilet water tank (covered with cardboard). Instructed person in charge to replace and maintain lid with cleanable material.

55. Physical facilities installed, maintained & clean
- comments: observed ceiling tiles in rear of facility with water damage and brown staining. Instructed person in charge to replace and maintain.

58. Allergen training as required
- comments: observed certified manager on duty with no allergen training. Instructed to obtain training and retain certificate on site at all times.
