- comments: observed employee touch raw meat then proceed to give bowl to another employee to pour soup without washing hands. Instructed manager to ensure that employees who touch raw animal products wash hands before touching other food contact surfaces. Manager instructed employee to wash hands. Priority violation 7-38-010 citation issued.

- comments: observed no correct parasite destruction agreement between establishment and supplier on site during inspection. Instructed manager to obtain parasite destruction agreement from supplier stating that parasites are frozen to a temperature and for a time or an agreement from supplier stating fish were commercially raised and fed. Priority foundation violation. Citation issued. Violation 7-38-005
33. Proper cooling methods used; adequate equipment for temperature control
- comments: observed cooler with non tcs foods with an internal temperature of 49 f. Instructed manager to ensure all coolers have an internal air temperaure of 41 f and below. Manager adjusted cooler. Priority violation 7-38-005. No citation issued.

- comments: observed openings at bottom and side of rear door in rear dish area. Instructed managerto repair and maintain to prevent pest entrance.

- comments: observed approximately 150-200 rodent droppings in rear storage hallway along walls, bottom of table and on top of boxes and containers. Instructed manager to clean, declutter and organize boxes and to service area. Pest control service recommended. Priority foundation violation 7-38-020(a) citation issued.

- comments: observed wiping cloth laying on food prep counter. Instructed manager to place wiping cloth in container of sanitizer. Instructed manager surfaces need to be sanitized in between uses.

- comments: observed toaster oven in front prep area heavily soiled on the outside instructed manager to clean and maintain.
52. Sewage & waste water properly disposed
- comments: observed waste water being improperly disposed to floor. When rear hand washing sink is used water floods floor from drain. Instructed manager to ensure waste water is adequately disposed. Plumber arrived to adress issue. Priority violation 7-38-030(c) citation issued.

- comments: observed holes in ceiling in basement area above lamp and on ceiling near rear door of storage area.

- comments: observed previous core violations not corrected within specified timeframe. Report#2562246 dated august 16th 2022 instructed manager to label all food contatiners, detail clean flooors and walls, declutter unnecessary items in basement, replace missing lightshield in food prep area, keep up to date food handler records. Observed containers of salt and condiment items not labeled; observed build up of debris on floor and along walls in prep area and behind bar area, observed unnecessary items and clutter in basement; observed missing lightsheild in rear near ice machine, observed exipered food handler certificates. Instructed manager to address all previous core violations. Priority foundation violation 7-42-090 citation issued
