- comments: 6-202. 15 observed openings at bottom and side of rear door in rear dish area. Instructed manager to repair and maintain to prevent pest entrance.
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed toaster oven in front prep area with soil build up on the outside, accumulated grease and food debris on cooking equipment, shelving and exterior of food containers in food prep areas. Instructed manager to clean and maintain all.
55. Physical facilities installed, maintained & clean
- comments: 6-501. 11 observed damaged ceiling tile near rear door in corner. Instructed manager to replace.
59. Previous priority foundation violation corrected
- comments: observed previous priority foundation violations #60 not corrected within specified timeframe. Report#2574825 dated april 24 2023: observed more than 200 rodent droppings throughout facility including: along floors and shelving in dry storage area, in cabinets of deep fryers, along prep table in kitchen, and underneath booths in dining room. Observed dirt and debris build up along floors and walls throughout kitchen and front prep area. Observed clutter within basement. Instructed manager address all previous priority foundation violations. Priority violation 7-42-090 citation issued. Instructed facility to declutter, clean and service areas affected by rodent droppings, declutter basement, and clean among walls and floors throughout facility. Have premises serviced by pest control company.
