- comments: observed no chicago food sanitation manager on site or valid city of chicago certificate posted when tcs food prepared and served. Instructed to obtain city of chicago certification and retain on site. A chicago food sanitation manager must be on site and valid certificate posted at all times tcs food prepared and served. Priority foundation 7-38-012 citation issued.
10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no hot or cold running water at handsink in prep area. Instructed manager to repair and maintain. Must provide at least 100f at hand washing sink. Priority 7-38-030 (c) citation issued.

- comments: observed no thermometers in coolers. Instructed manager to provide operable thermometers in all coolers.
38. Insects, rodents, & animals not present
- comments: observed rear exterior door not to be tight fitting at bottom and around sides of door. Instructed manager to repair and maintain to be tight fitting at all times.

- comments: observed 20-30 mice droppings in rear prep warewashing area, observed 50-60 mice droppings in basement behind boxes and in between shelving. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a) citation issued

- comments: observed no chemical test strips on site for chlorine sanitizing solution. Instructed manager to obtain and retain on site for use. Priority foundation 7-38-005 citation issued.

- comments: observed no hot water at washbowl sink in washroom(88. 7f), observed no hot water a three compartment sink (91. 2f)in rear warewashing area, observed no hot water at utility sink (81. 9f). Instructed manager to provide hot water under city pressure, at least 100f at hand washing sinks and 110f at three compartment sink. Priority 7-38-030(c) citation issued.

- comments: observed build up of grime on toilet bowl and sink. Instructed manager to clean and maintain.

- comments: observed several places throughout facility with holes and spaces where wall adjoins floor. Observed broken wood thorughout facility in corners where floor adjoins wall.

- comments: observed several ceiling tiles to be missing, dirty, or damaged throughout facility. Observed broken wood hanging from ceiling in rear warewashing area. Instructed manager to repair, replace, and maintain.

- comments: observed no light fixture in ceiling in rear warewashing area. Instructed manager to provide and maintain. Observed no light in basement area, instructed manager to provide and maintain.

- comments: observed food handler preparing tcs foods with no food handler training. Instruced manager to comply with the idph food handler requirement for all employee's handling tcs foods.

- comments: observed several customers, as well as two employees not wearing mask while on premises for inspection. Instructed manager that mask must be worn at all times inside of establishment in compliance with city of chicago health orders. Public health order 2-112-340 citation issued
