- comments: observed no hot water at hand washing sinks in staff/dining washrooms(75. 3f) and second floor women washroom (72. 5f). Instructed manager to provide at least 100f water at all hand washing sinks. Priority violation 7-38-030(c) citation issued

- comments: observed the following doors not rodent proofed, south entrance doors 1' gap between doors, main entrance doors 1' gap on bottom left door, north entrance doors 1' gap betweeen doors. Instructed manager to seal all outer openings on doors to prevent pest entry.

- comments: observed milk crates used as shelving units in storage areas. Instructed manager to provide adequate shelving for proper cleaning and pest control.
