- comments: no certified manager onsite during the inspection while tcs foods; eggs, ham, and chicken, are being prepared or served. Instructed to have a certified manager onsite at all times when tcs foods are being prepared or served. Priority foundation 7-38-012 citation issued.

- comments: must provide date markings to tcs/rte foods, prepared onsite and held in coolers over 24 hrs. Instructed to provide consume by dates and maintain. Priority foundation 7-38-005 no citation issued.

- comments: observed 5 live roaches in front prep area, also observed 30 rodent droppings in the rear storage room adjacent to the food prep area.
