- comments: no paper towels provided at rear handwashing sink in prep/dishroom. Paper towels provided upon my request priority foundation: 7-38-030(c),consolidate with above violation

22. Proper cold holding temperatures
- comments: found cooked rice and shrimp rooled up at temp of 56. 5f,rolled up cooked rice with pork at temp of 45. 3f, cooked pork buns 91. 4f to 86. 0f, food stored inside the self servicing cooler. Fried pork at temp of 70. 2f, stored on top front counter; cold food must maintain temp of 41f and below. Food discarded and denatured. Priority violation: 7-38-005,citation issued

39. Contamination prevented during food preparation, storage & display
- comments: splash guard is needed between counter prep and handwashing sink at front area by the ice machine. Surface must be smooth,cleanable and non-absorbent material

41. Wiping cloths: properly used & stored
- comments: must store wiping cloths in prep areas in sanitizer bucket between uses to prevent cross contamination. Instructed to maintain.

- comments: ice scoop must be stored in a clean container between use. -

- comments: must not store clean cutting board on the back-splash of the three compartment sink ,relocate to clean shelf

- comments: observed plastic food containers on shelves splashed with encrusted blood,in rear hallway. Instructed to clean and maintain. Shelves inside walk-in cooler with encrusted debris,clean and maintain.

- comments: broken cold handle at staff washroom. Repair cause

- comments: ceiling inside the walk-in cooler with peeling material by the condenser unit,instructed to clean and maintain

- comments: employees bag,sweater shoes etc stored at rear prep area on shelves,instructed to provide a separate area/locker for employee personal items. No employee personal items must be stored in prep area.

60. Previous core violation corrected
- comments: previous core violations not corrected from 6-26-2021,report #2382344. #37) 3-302. 12 found bulk food not labeled. (flour, rice,etc. )must label. #51) 5-204. 12 found no backflow prevention devices at ice machine waterline. Must provide where it can be located and serviced. #55)6-501. 12 found floors not clean at preparation areas(grease buildup). Must clean. Priority foundation violation:7-42-090, no citation issued.
