10. Adequate handwashing sinks properly supplied and accessible
- comments: soap and disposable towels stocked at utility/mop sink in washroom. Must remove hand washing aids from utility/mop sink.

21. Proper hot holding temperatures
- comments: observed cooked beef and cooked chicken at improper temperatures of 60. 6f-109. 7f inside of the hot holding steam table at front prep area. Management voluntarily discarded approximately 10's of food worth $450. 00. Instructed manager, (tcs foods) must maintain cold holding temperature of 41. 0f or colder and hot holding temperature of 135f or hotter at all times. Priority violation 7-38-005,citation issued.

35. Approved thawing methods used
- comments: observed improper thawing being done on frozen chicken. Thawing must occur with food being placed in the cooler over night, or under cold running water. Instructed to maintain correct thawing procedures.

38. Insects, rodents, & animals not present
- comments: observed sticky fly trap roll hanging above the rear prep/dishwashing area. Instructed to remove and maintain.

- comments: observed pest infestation in the premises. At this time of inspection observed over 200 rodent (mice) droppings throughout the premises, along walls and in all corners of the kitchen prep area, and rear prep/dishwashing/storage area on the bottom of storage racks, in the interiors of motor housings of the refrigeration units on top and around the rear furnace area. Instructed to detail clean and remove all rodent (mice) droppings and sanitize all affected areas. Pest control service is needed to eliminate the pest activity. Priority violation 7-38-020(a), citation issued.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed stained cutting board on top of the rear table in the rear prep/dishwashing area. Instructed to resurface or replace the cutting boards and maintain.

49. Non-food/food contact surfaces clean
- comments: instructed to detail clean kitchen storage racks, prep tables, food storage containers, tops/bottoms of prep tables and prep coolers, cooking grill station, interior bottom compartment of the deep fryer. Remove all grease, food debris etc.

51. Plumbing installed; proper backflow devices
- comments: observed water leak at drainpipe underneath the dishwashing 3-compartment sink area. Instructed to repair plumbing leak and maintain.

52. Sewage & waste water properly disposed
- comments: observed wastewater backing up at the dishwashing 3-compartment sink when the water is released, or run, from middle compartment, water backs up into the left and right compartments of the 3-compartment sink and water slowly draining. Instructed to repair plumbing issue. Priority violation 7-38-030 (c), citation issued.

55. Physical facilities installed, maintained & clean
- comments: observed taped (attached) glue boards on walls along the furnace equipment and rear wall located in the rear prep/dishwashing area. Instructed to remove said glue boards and maintain so the walls are repaired to smooth, easily cleanable, non-absorbent surfaces. Also, instructed to detail clean food splashed debris on walls/ceiling at rear prep/dishwashing area and maintain.

55. Physical facilities installed, maintained & clean
- comments: observed food debris and spills on the floors and surfaces throughout the premises. Instructed to detail clean under all shelving units, interior of prep/ dishwashing area,prep tables and all cooking equipment to remove spills, food debris, and dirt buildup. Instructed to detail clean and maintain at all times.

55. Physical facilities installed, maintained & clean
- comments: instructed to detail clean and organize the rear prep/dishwashing area and remove unnecessary articles stored on top of the rear refrigeration units due to potential pest harborage and elevate cleaning equipments off the floor at least 6" inches to facilitate cleaning. (no milk crates to be used for elevation or storing of items). Also, must remove from the premises all piled up wooden broken chairs/grill/litter stored directly outside the rear exit door.

56. Adequate ventilation & lighting; designated areas used
- comments: instructed to detail clean the kitchen exhaust hood/filters to remove accumulation of grease build-up and maintain.

57. All food employees have food handler training
- comments: observed a food handling employee with expired food handler certificate. Instructed to show proof of training for all food handling employees and maintain as required.
