- comments: tcs food at improper temperature: 10lb of soft shredded cheese at temp of 55. 7f,2lb of cooked chicken at temp of 49. 5f. Cold food must maintain a temp of 41f and below. Food stored inside the prep cooler priority violation: 7-38-005, citation issued

- comments: observed one employee without hair restraint in the prep area. Instructed to provide.

- comments: previous core violations not corrected from 8-25-2020,report #2386480 #39)3-305. 12 noted containers of assorted food items stored underneath the waste water pipe lines at the basement storage area. Instructed to remove or relocate items from under pipes or encase pipes. #55)6-501. 11 noted a large holes, cracks and openings on wall through out the basement food prep/storage area, peeling paint on walls all at the basement food prep/dry storage. Instructed to repair and maintain, seal all holes and openings to prevent hiding places for pest. #55)6-201. 11 instructed to detail clean and maintain floors along wall base and underneath shelving at the basement food/dry storage area. Must also clean and sanitize area with mice droppings. Priority foundation violation:7-42-090,no citation issued.
