- comments: 6-202. 13 observed insect control device being placed by the facility. Instructed to not place any pest devices, only pest control can handle all pest devices.

- comments: 5-204. 12 observed an inadequate air gap provided on the ice machine. Instructed to provide at least a 6 inch air gap between the floor drain and pipe that is connected to the ice machine.

- comments: 6-201. 11 observed walls not covered behind the 2-door refrigerator and the soup unit located across from the walk-in cooler/freezer. Observed floors need to be re-grouted by the 3-compartment sink. Observed some cracked floor tiles. Instructed to make all surfaces smooth and easily cleanable.

- comments: 6-501. 12 observed excessive flour, food, grease, and dirt debris under all cooking units, around all refrigeration units, and under and around the front customer counter/food prep. Observed standing water under the 3-compartment sink. Instructed to clean all.
