- comments: observed incorrect employee health policy on site. Must provide written employee health policy (with norovirus information) for all employees. Priority foundation violation. 7-38-010.

25. Consumer advisory provided for raw/undercooked food
- comments: observed consumer advisory for raw or undercooked foods not correct on menu. Instructed to identify such foods by asterisking them to a foot note that states: 'items can be served raw or undercooked or contain undercooked ingredients' and add a statement that says: 'consuming raw or undercooked animal products may increase your risk of food borne illness, especially if you have medical conditions'. Must provide disclosure, reminder and footnote on menu for items such as eggs and hamburgers. Priority foundation violation 7-38-005.

38. Insects, rodents, & animals not present
- comments: observed approximately 11o small, black flies in the following areas: in bars, in rear utensil storage hallway, on walls and ceilings of dish area, and on walls behind soda storage area. Must detail clean and sanitize these areas. Must contact pest control service and minimize pest issue. Priority foundation violation 7-38-020(a). Citation issued.

- comments: observed dish machine on on the left (not in use at this time) in bar on 1st floor not maintained. Dish machine registers 0 ppm of chemical sanitizer. Machine tagged and held for inspection. Must maintain all equipment. Must provide chemical sanitizer. Priority foundation violation 7-38-025.

- comments: observed excessive grease accumulation in lower interiors of fryers (in front cook line, and in rear cookline). Must detail clean and sanitize. Must maintain.

- comments: observed left toilet does not flush in ladies restroom on 1st floor. Must repair and mainta same.

- comments: observed soiled walls and ceilings in dish area and equipment/utensil storage areas. Must detail clean and sanitize to prevent pest harborage. Must maintain same.
