- comments: no certified food service sanitation manager certificate on-site while open foods are being prepared and served. Instructed to provide at least one certified food service sanitation training and certificate for manager present on-site at all times and maintain. Priority foundation violation. 7-38-012 citation issued

3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: no employee health policy on site at time of inspection. Instructed manager to provide and maintain for all employee. Priority foundation 7-38-010

5. Procedures for responding to vomiting and diarrheal events
- comments: observed no vomiting and diarrheal event clean-up kit on-site. Manager unaware of contents or requirement of vomit and diarrheal clean-up kit. Instructed to provide vomit and diarrheal cleanup kit and maintain on-site at all times. Priority foundation violation. 7-38-005

- comments: observed tcs food items out-of-temperature in the front deli display cooler, turkey at 46. 0f and ham at 44. 8f. Instructed to discard and maintain all tcs food items at 41. 0f or below. Pic discarded out-of-temperature food items. Approx value $30. 00 5 lbs priority violation. 7-38-005 citation issued.

23. Proper date marking and disposition
- comments: no evidence of any date markings on any open tcs food products instructed manager on how to properly date mark open processed food items. Priority foundation 7-38-005

28. Toxic substances properly identified, stored, & used
- comments: observed open bait in 1 reach in cooler that is in poor repair. Observed a plate with bait on it in rear storage area. Instructed manager to not have open bait and to have only pest control administer any pest control. Priority violation 7-38-015 citation issued

36. Thermometers provided & accurate
- comments: no stem therometer on site to properly take internal temperatures of tcs food products. Instructed to provide thermometer and maintain. Priority foundation 7-38-005 citation issued

38. Insects, rodents, & animals not present
- comments: oserved approximately 5 dead cockroaches in rear storage area and 5 dead cockroaches in front deli area beneath cold holding equipment. Instructed to remove dead insects and other pests from the facility and maintain.

43. In-use utensils: properly stored
- comments: observed spatula with handle stored in food items in front deli cooler. Instructed to remove utensil from food items and store on clean, sanitized surface when not in-use.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed ice cream freezer missing half of sliding door. Plastic bags used to cover exposed portion of freezer. Instructed to repair or replace freezer door with door that is tight-fitting and propetly-adjusted.

48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed no chemical sanitizer concentration test strips on-site. Instructed to provide chemical sanitizer concentration test strips for appropriate sanitizer and maintain on-site at all times. Priority foundation violation. 7-38-005 citation issued.

49. Non-food/food contact surfaces clean
- comments: observed soil and debris on exterior of all cooking and cold-holding equipment throughout the front deli area. Instructed to clean and sanitize all non-food contact surfaces frequently to prevent soil buildup.

52. Sewage & waste water properly disposed
- comments: observed sewage backup at 3-compartment sink in front deli area. Water fails to drain and comes up other basin drains. Observed sewage coming out of 3-compartment sink basin drains. Instructed to repair 3-compartment sink to eliminate sewage backup and allow water from 3-compartment sink to drain properly. Priority violation. 7-38-030(c) citation issued.

55. Physical facilities installed, maintained & clean
- comments: observed debris and soil on floors, walls, and ceilings throughout facility. Instructed to clean and sanitize all floor, wall, and ceiling surfaces throughout facility at an adequate frequency to avoid soil buildup and maintain.

56. Adequate ventilation & lighting; designated areas used
- comments: observed un-shielded and non-shatterproof light bulbs above front deli area. Instructed to provide shielding for light fixtures or shatterprof bulbs for this area.

57. All food employees have food handler training
- comments: observed no food handler training certificates on-site. Instructed to provide food handler training and certificates to all employees and maintain on-site at all times.
