- comments: 2-101. 11: **pf** responsibility -- assignment (pic) observed facility food handlers preparing and handling time and temperature control for safety(tcs) foods without original city of chicago food service manager and certificate on site. Informed person in charge of the city of chicago original food service certificate must remain posted on site and a certified manager must be on site during tcs food prep, cooking, serving, and operating hours. Priority foundation violation #7-38-012. Citation issued.

- comments: observed previous priority foundation violations not corrected from inspection report#2621558 dated for 7/31/25 within specified time frame. (#3) 2-102. 14-food observed no written employee health policy on site. Instructed facility to establish an appropriate employee health policy and maintain with verifiable signed copies on site for all food employees in acknowledgment of reporting agreement. (#25) 3-603. 11: observed no disclosure and reminder on the printed menu for all animal food products that can be ordered raw or undercooked. Instructed to provide a disclosure on the menu for all items (eggs any style, steak, beef burgers, and brunch menus, etc. ) that can be ordered undercooked or raw and add a reminder on the menu that reads: *these items can be ordered undercooked or raw. Consuming undercooked or raw foods can increase your risk of foodborne illness. (refer to cdph consumer advisory website) (#38) 6-501. 111- observed pest activity with approx. 25 small black flies noted during inspection. Small fly activity was noted near the beer garden door and throughout the bar. Instructed to consult pest control, as additional services are needed to minimize or eliminate activity before re-inspection. (48) 4-302. 14: sanitizing solutions, testing devices observed facility without a quat chemical test kit for sanitizer for bar ware washing area. I informed the person in charge that a test kit that accurately measures the concentration in mg/l of sanitizing solution must be provided. (60) 8-404. 13(b:4): enforcement by the regulatory authority (non-compliance) previously citied core violations from 04/11/2024 report#: 2592594 not corrected: #58 2-102. 13 no allergen training. Must provide. Must provide certificates of completion on site and maintain records. Instructed person in charge that the violation shall now classified as a priority item due to previous noncompliance. Must correct before re-inspection. Priority violation #7-42-090. Citation issued.
