Report date: Jul 12, 202216. Food-contact surfaces: cleaned & sanitized
- comments: observed the low temperature dish machine not properly sanitizing at 0ppm of chlorine during the inspection. Instructed manager said equipment must properly sanitize at 50ppm-100ppm of chlorine all times. Unit tagged and held for inspection by the department of health and may not be used. Priority violation. Citation issued 7-38-025.
37. Food properly labeled; original container
- comments: observed pepper, salt and bread crumbs out of the original containers and in stainless steel containers by the prep station without labels containing the product name. Instructed to label all foods and spices with common name that are out of the original containers.
39. Contamination prevented during food preparation, storage & display
- comments: observed food items stored on floor in dry staorage area. Instructed manager to elevate all food six inches off floor.
41. Wiping cloths: properly used & stored
- comments: observed soiled wiping cloths stored on prep tables. Instructed manager to store in sanitizing solution between uses.

- comments: observed broken door handle on the two door cold holding. Must repair or replace broken door handle.

- comments: observed no irreversible thermometer on site at time of inspection for measuring the utensil surface temeperature. Instructed to provide. Priority foundation violation 7-38-005. Citation issued.

- comments: observed accumulated grease and food debris on interior and exterior surfaces of fryer. Instructed manager to clean and maintain.
51. Plumbing installed; proper backflow devices
- comments: observed cold water handle does not work on handwashing sink in 1st floor kitchen. Must repair and maintain same.
56. Adequate ventilation & lighting; designated areas used
- comments: observed multiple missing lightbulbs above 6th floor coffee area. Instructed to install working lightbulbs and maintain.
57. All food employees have food handler training
- comments: observed 2 employees with expired food handler certificates. Instructed all employees must complete food handler training and maintain records thereof on site at all times
58. Allergen training as required
- comments: observed no allergen training on site at time of inspection. Instructed all employees holding city of chicago foodservice manager certificates must complete allergen training and maintain records thereof on site at all times.
