- comments: observed corn meal in clear bin underneath rear prep table and flour underneath prep table near cooking equipment not labeled. All bulk ingredients that are not in original container must be labeled.

- comments: observed no splash guard on front prep handwashing sink. Instructed to provide to prevent contamination of food/utensils from hands.

- comments: obseverved wet wiping cloth near front handwashing sink. Instructed to place wiping cloths inside sanitizer bucket when not in use.

- comments: observed no sanitizing test strips on site for checking chemical concentration (chlorine)ppms at 3 compartment sink. Instructed to provide and maintain at all times. Priority foundation violation. 7-38-005. Citation issued.

- comments: observed food residue in bottom of standing stainless steel freezer. Must clean and maintain.

51. Plumbing installed; proper backflow devices
- comments: observed leak underneath front handwashing sink and slow draining water. Must repair and maintain.

- comments: observed food debris, dirt, and other soil on floors throughout facility. Instructed to clean and maintain.

- comments: no proof of food handler training on premises. All food handlers must obtain food handler training within 30 days of hire.

- comments: previous core violations from report dated 06/15/2021 not corrected: #36 observed no thermometers inside of all of cooling units.
