- comments: observed a dead rat behind the fryers in the kitchen. Must remove and detail clean area.
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed rusted meat hooks inside the walk in cooler. Must replace and maintain.
49. Non-food/food contact surfaces clean
- comments: must detail clean front storage shelving, interior of the walk in cooler to remove debris. Also must detail clean the interior of the ice machine in the basement. Accumulated food debris on wheeled carts, cooking equipment and food containers. Must clean and maintain all.
50. Hot & cold water available; adequate pressure
- comments: observed the hand sink in the basement wash room in poor repair. Must provide and maintain hot and cold running water under city pressure as required. Priority violation 7-38-030(c). No citation issued.

- comments: observed the grease trap in the basement dirty. Must clean and maintain greasetrap and surrounding surfaces.
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: instructed manager to provide and maintain a garbage can with a cover in the women washroom as required

- comments: observed the wooden floors inside the walk in cooler encrusted with food debris and spills. Floors must be smooth and easily cleanable and kept clean.

- comments: must detail clean floors through out, including basement. Especially where droppings were found. Also must open area below the carnitas cooking station and clean and maintain free of debris.

- comments: observed a few holes and openings in to the wall along the base board in the rear prep area. Openings in pipes in basement areas. Must smoothly seal all openings to prevent entry or harborage points for pests.

- comments: observed clutter through out the basement, kitchen and dish washing areas. Must remove and organize to prevent harborage. Also must be able to properly monitor for pest activity.

- comments: observed the city certified manager without the required allergen training certificate. Instructed to comply or citations will follow.
59. Previous priority foundation violation corrected
- comments: previous priority foundation violation from 11/30/21 inspection not corrected. #38: still observed rat droppings through out premises: 15 rat droppings scattered on the shelf and floor underneath the hot holding unit in the front, 50 rat droppings on the floor where mops and dry goods are stored next to front 3 compartment sink, 10 droppings scattered on the floor under the rear 3 comp sink, 2 droppings in the furnace room, 10 rat droppings behind the fryer and 1 rat dropping inside the fryer cabinet, 5 rat droppings on basement floor. Priority violation 7-42-090. Citation issued. Must remove all rodent droppings, clean and sanitize surfaces and equipment. Have premises serviced by pest control operator and eliminate rodent activity.
