- comments: the person in charge does not have a city of chicago certified food manager certificate.
2. City of chicago food service sanitation certificate
- comments: no city of chicago certified food manager on the premises while tcs foods (pork, chicken, rice, seafood, etc. ) were being prepared and served. Instructed management that a city of chicago certified food manager must be present at all times while tcs foods are being prepared or served. Priority foundation violation, 7-38-012. Citation issued.
10. Adequate handwashing sinks properly supplied and accessible
- comments: must provide handwashing signage at the handwashing sink in the rear prep area next to prep sink behind city grill hot foods sections.
14. Required records available: shellstock tags, parasite destruction
- comments: observed premises not maintaining record keeping system for shellstock tags. Shellstock tags must be stored in chronological order and kept on site for at least 90 days. Priority foundation violation, 7-38-005. Citation issued. Pic and food employee organized and revised shellstock tags. -cos.
16. Food-contact surfaces: cleaned & sanitized
- comments: observed low temperature dish machine in the dishwashing area not properly sanitizing with chlorine reading 0 ppm. Instructed to use the 3-comp sink to wash, rinse, and sanitize all dishes and utensils until dish machine has been repaired. Priority violation, 7-38-025. Citation issued. Auto-chlor arrived on site at the time of inspection to maintanence dish machine. Dish machine is now maintaining 50 ppm. -cos.
38. Insects, rodents, & animals not present
- comments: observed multiple dead insects on glueboards in the produce room under prep table. Instructed to replace glueboards and clean all dead insects from the premises.
43. In-use utensils: properly stored
- comments: observed multiple scoops used for hot food items stored in dipper wells with stagnant water in the hot foods sections. Instucted to provide running water for all dipper wells and maintain.
45. Single-use/single-service articles: properly stored & used
- comments: observed cardboard trays in the crispy town hot food section stored facing up on the prep table behind hot holding station. Instructed management to invert all food storage containers to protect from overhead contamination.
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed torn gaskets on the inside of three door prep cooler in the rear prep area behind grill city. Instructed to repair and maintain.

- comments: observed cutting boards with open seams and dark stains in the meat/seafood cutting room. Must refinish or replace damaged or stained cutting boards and maintain.
51. Plumbing installed; proper backflow devices
- comments: observed a leak from the pipe underneath handwashing sink in the hot foods dishwashing area, and a slow drainage at the handwashing sink in the grill city front prep area. Instructed to repair both handwashing sinks and maintain. Plumber arrived on site at the time of the inspection and sinks were repaired. -cos.

- comments: observed all wastecans inside of the womens customer restroom without a lid. Instructed to provide wastecnas with lids for feminine hygiene products.
