1. Person in charge present, demonstrates knowledge, and performs duties
- comments: observed person in charge without proof of food safety management training during time of inspection. Must provide.

2. City of chicago food service sanitation certificate
- comments: observed no valid city of chicago food service manager and certificate on site. Noted food handlers preparing and handling time and temperature control for safety(tcs) foods (seafood, cooked potatoes, cooked broccoli). Informed person in charge city of chicago original food service certificate must remain posted on site and certified manager on site during tcs food prep, cooking, serving and operating hours. Priority foundation violation 7-38-012. Citation issued.

5. Procedures for responding to vomiting and diarrheal events
- comments: observed no adequate supplies (disposable mop, disposable latex gloves, disposable mask, and adequate sanitizer *must eliminate norovirus*) and clean up procedure on site to properly clean up any vomit or diarrhea events that can potentially occur inside of the food establishment. Instructed to provide adequate supplies for vomit and diarrhea clean up. Priority foundation violation 7-38-005. Citation issued.

35. Approved thawing methods used
- comments: observed facility not meeting thawing requirements. Food handler thawing frozen sausage on 3 compartment sink drain board sitting in stagnant water. Informed food handler of proper thawing methods (under running water with enough volatility or placed in walk-in cooler). Must meet proper thawing requirements and maintain

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed raw wood used for resurfacing of flooring in walk in cooler. Noted heavy food debris splatter on wood and uneven flooring (rust and erosion scraps covered by black pallets in walk in cooler. Instructed to restore to a smooth even surface, deep clean and seal wood.

- comments: observed leak from sanitizing basin of 3 compartment sink. Instructed to repair and maintain

52. Sewage & waste water properly disposed
- comments: observed inadequate waste and sewage water disposal from 3 compartment sink. Noted water and food debris coming up and spilling over to floor when sanitzing basin was released. Must provide proper drainage from sink. Priority violation 7-38-030(c). Citation issued. "
