Report date: Oct 22, 202110. Adequate handwashing sinks properly supplied and accessible
- comments: observed no hand washing soap/cleanser at the hand washing sink located next to 3 compartment sink. Instructed to always maintain hand washing cleanser at all hand washing sinks at all times. Employee provided hand washing cleanser during the inspection. Priority foundation. Citation written under 6-301. 12
10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no paper towels or sanitary hand drying devices at hand washing sink next to 3 compartment sink. Instructed manager to provide. Priority foundation. Citation issued 7-38-030(c).
22. Proper cold holding temperatures
- comments: observed tcs foods held at improper internal temperatures in the front prep cold holding unit, such as 5 lbs raw chicken at 48. 5f, 6 lbs of shrimp at 48. 2f, 8 lbs of fish ranging from 51. 6f to 52. 5f. Manager discarded and denatured a total 19lbs at $400. All cold tcs food must maintain 41f or below. Priority violation. Citation issued 7-38-005.
33. Proper cooling methods used; adequate equipment for temperature control
- comments: observed front 3 door prep cooler not maintaining proper temperature 50. 1f. Instructed manager to repair and maintain cooler at 41f or below. Manager cleaned coils underneath the cooler and temperature is holding at 37. 4f. Priority 7-38-005 citation issued.
36. Thermometers provided & accurate
- comments: observed no ambient air thermometers inside 3 door reach in cooler. Instructed to provide conspicuous and easily readable thermometers inside all refrigerator units to properly monitor the ambient air temperature of equipment.
37. Food properly labeled; original container
- comments: observed bulk plastic bins of flour/dry breading without labels. Instructed manager to label all foods with common name that are out of the original containers.
39. Contamination prevented during food preparation, storage & display
- comments: observed attached to wall soap dispenser above prep table with hot holding unit for cheese sauce and wing sauce. Instructed manager to either relocated the soap dispenser or relocate hot holding unit to prevent soap from contaminating any food.
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed aluminum foil used as liner on shelving units in prep areas. Instructed manager to remove all foil from shelving units. All food and non-food contact surfaces must be smooth and easily cleanable.
49. Non-food/food contact surfaces clean
- comments: observed accumulated grease and food debris on interior and exterior surfaces of cooking equipment, prep tables, coolers, freezers, shelving units. Instructed manager to clean and maintain. (nonfood contact surfaces)
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no covered receptacles for feminine hygiene products inside of staff toilet rooms. Instructed to provide and maintain.
55. Physical facilities installed, maintained & clean
- comments: observed unclean floors throughout premises, soiled unclean cardboard used as a floor runner at fryer door. Observed broken floor tiles at entrance at walk in freezer. Instructed to discard, clean and repair.
57. All food employees have food handler training
- comments: observed no food handlers certificates for male employee handling food. Instructed manager to comply with the idph food handler requirement.
64. Public health nuisance
- comments: observed male employee at fryer without face covering. Instructed manager that all employees must comply with mandate. Citation issued 7-28-060.
