5. Procedures for responding to vomiting and diarrheal events
- comments: found no procedure/plan for responding to vomiting and diarrheal events. Instructed to provide and maintain required procedure/plan and appropriate supplies or kit. (necessary items at a minimum: gloves, face masks, disposable mops and appropriate sanitizer/spray that is effective against norovirus). Priority foundation violation 7-38-005.

- comments: observed no handwashing signage in 1st floor washroom. Must provide and maintain.

- comments: instructed to properly label bulk food containers/bins identified with common name (salt, sugar, flour etc).

- comments: observed 3 live cockroaches on floor in between dining booths at front customer dining area and 2 live cockroaches on wall next to the spice rack in kitchen food prep area. Instructed to detail clean and remove pest and sanitize affected areas;additional pest control service is now reccomended. Priority foundation violation 7-38-020(a),citation issued.

- comments: instructed to remove glue boards with dead insects behind fron service counter and behind refrigeration units in kitchen food prep area and in furnace room in basement.

- comments: observed frozen foods stored in plastic grocery bags inside of the rear chest freezer. Instructed to use only food grade storage bags or containers.

- comments: observed chipped paint and rust on the wire storage racks in the walk-in cooler. Instructed to remove chipped paint and rust and repaint or replace the shelves.

- comments: instructed to detail clean tops and sides of the cooking equipments and kitchen storage shelfs and interiors of coolers to remove accumulated food debris/grease build-up and maintain.

- comments: floors along the walls and in all corners throughout the kitchen food prep area and rear dry storage room requires a detail cleaning and walls throughout the kitchen prep and 3-compartment sink area to remove food splashed debris and also light shields in ktchen food prep area need cleaning to remove debris.

- comments: observed exposed brick walls in rear dry storage areas. Instructed to seal such that mentioned walls are smooth,easily cleanable surfaces and seal noticeable openings on crevices on walls near the front service counter and at ceilings around pipes near the cooking equipments in kitchen food prep area.

- comments: instructed to detail clean kitchen exhaust hood/filters to remove excessive grease build-up and maintain.

- comments: previous core violations not corrected from inspection report#2484832 dated:02/17/2021 #47 observed cutting board on main cook line covered in deep grooves and dark stains. Instructed to repair or replace and maintain smooth, cleanable surfaces. 05/08/2021 #47 observed torn gasket on prep cooler located opposite flat top grill. Instructed to repair or replace and maintain in good conditon. 05/08/2021 #51 observed pipe underneath exposed hand sink on main cook line in prep area repaired with plastic tape. Instructed to remove and repair with durable materials. 05/08/2021 instructed to correct noted violations and maintain by 11/24/2021. Priority foundation violation 7-42-090.
