38. Insects, rodents, & animals not present
- comments: observed 2 live roaches in the following areas: groove of the hood and in the three compartment sink behind the bar. Also, observed approx. 125 rat droppings in of the benches of the seating area and behind the benches of the booths of the seating area. Management instructed to clean and remove all activity from all affected areas. Additional pest control service is needed to eliminate the pest activity. Priority violation #7-38-020(a).

- comments: remove all dead insects from the following areas behind the bar, on the drink mats, in the 2-door sliding drink cooler, in the 2-door refrigerator in the kitchen, throughout the whole facility.

49. Non-food/food contact surfaces clean
- comments: observed soiled hood area in the kitchen. Instructed to detail clean all soiled areas. Clean the food debris and grease deposits inside of the ovens.

55. Physical facilities installed, maintained & clean
- comments: observed hole behind seating area bench in corner where black wall meets brick wall. Instructed to seal.

56. Adequate ventilation & lighting; designated areas used
- comments: clean and remove the dripping grease deposits on the edges of the ventilation hood above the cooking equipment.
